Academic Information


Please confirm that you are currently enrolled in a four-year college or university for classes in the upcoming academic school year. 


Enrollment in a community college (2-year) program is inelligible for scholarship through this program.



Please provide contact information for the college/university's Financial Aid Office, including the contact's name, email, and phone number.



To expedite scholarship payment, please provide the address of the location and/or person where your college/university accepts scholarship payments (i.e. Financial Aid Office, Enrollment Office)


Please confirm with your school if there is a deadline date that your college/university stops accepting external scholarship submissions for the upcoming school year. If yes, please provide the date below.



High School Information


Enter details below about your high school career, including:

-Description of work experience (paid or internships) during high school should include year, organization, responsibilities, hours/week, and indication of whether you were paid for your work.

-Describe extracurricular activities during each of your high school years.

-What leadership positions have you held at school and in your extracurricular activities?

-What Advanced Placement (AP) courses have you taken, or are taking currently? Please include the year and course taken.



Essay Section


In the fields below, please provide the following:

-Describe activities you have shared with your mentor. How will your experiences with her help you achieve your life goals?

-What are your hopes regarding attending college and earning your college degree?

-What do you think your greatest challenges will be as a college freshman?

-Please add any other information you feel we should know. (optional)


 

Documentation


Please upload the following documentation (PDF recommended)

-The Award Letter detailing your financial aid package from the college you will attend in the fall

-The latest Transcript of High School Record including fall semester of senior year. For your protection, please block out all but the last four digits of your social security number if it is present.


When documentation is not available at the time of application, you may send the award letter and transcript to [email protected]. All documents must be sent electronically and not by mail. All documentation must be received by July 8, 2024.
